Access your patient's health record

GPs and health professionals can access patient health records through the Health Provider Portal or our Information Access Units.

Health Provider Portal

You can view your patient's health care records using the online Health Provider Portal (HPP). If you don't already have access, you'll need to register

Records you can view include:

  • pathology reports
  • discharge summaries
  • medical imaging reports
  • medicine list
  • outpatient letters
  • operation reports
  • test results.

My Health Record

You may be able to access patient records if your patient has a My Health Record on the digitalhealth.gov.au website.

Information Access Units

You can also request a Release of Information (ROI) to patient information.

Your request must be done using your practice letterhead and include the:

  • full name and date of birth
  • specific and exact information, for example the specialty and year
  • signed consent from the patient.

Due to the high volume of requests we get every day, we don’t provide complete or a full copy of records using the ROI process.

Update the Secure Transfer Service (STS) Address Book

We use the STS Address Book to:

  • send patient records to GPs
  • get referrals from GPs 
  • support GP registration to the HPP.

You need to update the STS Address Book every time there's a change to staff members or provider information at your practice.

Update your practice details
  1. Fill in the STS Address Book form
  2. Email as an attachment to STS_External-Alerts@health.qld.gov.au
